This was in fact done for a coursework module, in which I was required to design a book cover for Roald Dahl's short story 'Skin'. The illustration you see is the main illustration that featured on the front cover. The banana skin is of course a hint at the title, however is mainly a depiction of the macabre humor aspect of the story, the banana skin being a symbol of comedy, and the pool of blood a symbol of horror.
